What a groin lump usually turns out to be
In adults, a soft lump in the crease of the groin that was not there a few months ago is a hernia until proven otherwise. It happens when a weakness in the abdominal wall lets fat, or a loop of bowel, push through. The lump is the contents, not the hole.
Inguinal hernias account for the large majority. Femoral hernias sit slightly lower and more towards the inner thigh, are commoner in women, and matter more because the opening they pass through is narrow and rigid — they are far more likely to trap bowel.
Signs that point to a hernia
- It appeared gradually rather than overnight
- It gets bigger when you stand, cough, lift or strain
- It shrinks or disappears when you lie down
- It feels soft and can be pushed back in with gentle pressure
- There is a dragging ache rather than sharp pain
What else it could be
Not every groin lump is a hernia, which is why examination matters before anything is planned. Enlarged lymph nodes are firm, do not change with position, and often follow an infection in the leg or foot. A lipoma is soft but static. A saphena varix — a dilated vein — disappears when you lie flat and can mimic a hernia closely. In men, swellings arising from the testis or cord are a separate group entirely.
Distinguishing these is a clinical skill rather than a scan. Where there is genuine doubt, a dynamic ultrasound performed while you strain will usually answer it, and can be arranged alongside your consultation.

- Becomes suddenly and severely painful
- Turns hard, tender, red or dusky
- Cannot be pushed back in when it previously could
- Is accompanied by vomiting, or by not passing wind or stool
Does a groin hernia always need an operation?
Not on the day you find it. But hernias do not close by themselves, and the opening tends to widen over months and years. Long-term follow-up of men who chose to wait shows that most eventually come to surgery anyway, usually because of pain or a rising nuisance rather than an emergency.
The practical argument for repairing a hernia while it is small is that a small hernia is a simpler operation, with a shorter recovery and a lower chance of complications, than the same hernia three years later.
How repair works
Almost all groin hernias are now repaired keyhole or robotically, placing a flat mesh behind the muscle wall to reinforce the whole area rather than stitching the defect under tension. Most patients go home the same day, drive within a week to ten days, and return to desk work inside two weeks. Heavy lifting and gym work resume at four to six weeks.

Common questions
Can a groin hernia go away on its own?
No. The gap in the abdominal wall cannot heal closed while there is pressure pushing against it from the inside. The lump may come and go depending on your position, but the underlying defect stays and slowly enlarges.
Is a hernia dangerous if I leave it?
Most groin hernias are not dangerous in the short term. The risk that matters is strangulation, where trapped bowel loses its blood supply and requires emergency surgery. It is uncommon but it is unpredictable, and it is more likely with femoral hernias and with hernias that have a narrow neck.
How quickly can I have it repaired privately?
Surgery can usually be scheduled within 7 days of your clinic consultation, subject to theatre availability and your fitness for anaesthetic. Self-pay hernia repair starts from £4,500.
Do I need a scan first?
Usually not. Most groin hernias are diagnosed on examination alone. An ultrasound is reserved for cases where the diagnosis is genuinely uncertain, or where symptoms are present without an obvious lump.
Can I still exercise with a hernia?
You can, and exercise will not burst it. But heavy lifting, straining and high-impact work tend to make it larger and more symptomatic over time, and a hernia that is causing pain during exercise is a hernia that is telling you something.
