Strangulation — where trapped tissue loses its blood supply — is time-critical. Irreducible does not always mean emergency
Many hernias become chronically irreducible without ever being acutely dangerous. Adhesions form between the contents and the sac, and over months the hernia simply stops going back, while remaining soft, non-tender and otherwise unchanged. That is incarceration without obstruction or strangulation.
This group is not an emergency, but it is not a wait-and-see situation either. The safety margin has narrowed: the contents can no longer retreat, and the same hernia is now more likely to obstruct. Prompt planned surgery is the appropriate response.
Features that suggest chronic rather than acute incarceration
- It has been unreducible for weeks or months, not hours
- It is soft and not tender to press
- The skin over it looks entirely normal
- Your bowels are working normally
- You feel well in yourself
Should you try to push it back?
Gentle pressure while lying flat with the hips slightly flexed is reasonable if the hernia is not painful. Forceful attempts are not. Repeatedly pushing hard on a tender hernia risks reducing damaged bowel back into the abdomen, where a developing problem becomes far harder to recognise. If it does not go back easily and comfortably, stop and seek assessment.
